Prequalification Requirements

Get qualified to work with Anchor Construction.

For consideration on bids, we require the following information from our subcontractors:

EMR from the past three (3) years
OSHA 300 logs from the past three (3) years to include
- A list of OSHA violations during this time frame
- Composition of safety department and submission of company plan
Current number of employees
Copy of W-9
Certifications
- Disadvantaged Business Enterprise (DBE)
- Woman Business Enterprise (WBE)
- Minority Business Enterprise (MBE)
Recently completed and current projects to include
- Three (3) trade references from the company’s largest projects in the past three (3) years
Current financial statement to include
- A statement declaring the company has not or does not intend to file bankruptcy protection within the past five (5) years
- Yearly revenue from the past three (3) years
- Three (3) credit references
Surety information to include
- Bond information
Insurance information to include
- COI with current policy limits for general liability, auto, workers compensation, and umbrella
- Statement declaring company has not been debarred in the past five (5) years
- Explanation of any subcontractor terminations for default in the past five (5) years
- Explanation of termination of an insurance policy or refusal to renew a policy in the past five (5) years
- Statement declaring a lack of felony convictions for project managers and supervisors
